Job overview

As a Nurse (RMN)working in the Bedfordshire Dementia Intensive Support Service ( DISS) you will be responsible for undertaking detailed, holistic assessments of people living with dementia, or suspected dementia that are in crisis; or are at risk of a breakdown in care.You will work across Luton undertaking assessments, developing treatment plans and reviewing interventions by the DISS. You will be a role model to both paid and unpaid carers and will work with people in their own homes, caring environments and in reach into acute hospitals. You will be required to educate about the causes of distress and behaviour in dementia and you will be required to teach both paid and unpaid carers informally and through arranged formal sessions.This will require the post holder to work shifts covering the hours of 9am to 8pm, across a seven day working week, including bank holidays. The team have a four-hour response time for people that require an urgent response to assess their needs, prevent unnecessary hospital admission and breakdown in care and put management plans in place to minimize risk to the individual and others.

The Nurse (RMN) will be required to take a shift leader role, which will be identified on the rota, where you will be responsible for allocating staff and prioritizing response by undertaking effective triage.

1.1.Undertake assessments and develop robust treatment plans with service users and paid and unpaid carers within service users own homes, including 24hour care environments. You will inreach into acute hospitals and other environments to meet the needs of the service user.

Person specification

Education/ Qualification/ Training

Essential criteria

1. •Registered Nurse, Mental Health with current registration with NMC.
2. •Evidence of post registration continued professional development, qualifications or equivalent experience
3. •Educated to degree level or equivalent experience

Desirable criteria

4. •Practice Educator or mentor
5. •Independent and Supplementary Prescriber
6. •Dementia specific qualification

Experience

Essential criteria

7. •Previous experience of working within the community setting
8. •Previous experience of working with people with dementia in a variety of settings
9. •Previous experience of leading others and functioning as a team member
10. •Previous experience of organisational change
11. •Previous experience of working within the NHS and with allied professionals, internal and external stake holders

Desirable criteria

12. •Previous experience of delivering training

13. •Computer literacy
14. •Excellent organisational skills - able to prioritise own work and manage own time.
15. •Excellent oral and written communication and presentation skills and the ability to meet tight timelines
16. •Excellent skills of conflict resolution, negotiation and facilitation
17. •Acts upon service user and carer feedback and information to improve the way things are done taking personal responsibility for resolving
18. •Ability to build relationships and negotiate satisfactory outcomes with internal and external Stakeholders
