The Mental Health Practitioners play a crucial role in processing, assessing, and determining the mental health and social care needs of referred individuals. They assess ongoing needs and facilitate service provision. The role requires excellent risk assessment and management skills, including positive risk-taking and decision-making, to enable safe and effective responses for an identified client group, primarily via telephone/assessment.
Practitioners will participate in delivering the service's aims and objectives, identifying factors that may inhibit their achievement. They will assist in providing a high standard of mental health care to service users and implement systems to monitor care effectiveness.
The position is based with either the Shropshire or Telford Crisis Resolution Home Treatment Team, depending on preference. There may be some expectation to be based at The Redwoods Centre or Hall Court at times.
You will work closely with the Crisis team and the Access Call handlers.
The post involves working 22.5 hours per week, alternating weekly between:
Week one: Monday to Friday, 4pm - 8:45pm
Week two: Saturday and Sunday, 8am - 8pm
Main duties of the job
* Leading assessments for individuals referred to the Access Team.
* Developing and maintaining effective communication within the team and promoting positive working relationships with colleagues, departments, and other agencies.
* Organising and delivering informal, team-based training focused on the needs of clients of all ages with functional mental health issues.
* Managing time effectively to optimise staff and resource use when acting as Shift Co-ordinator, and informing senior team members of any issues impacting service delivery.
