To provide support to adults using Community Mental Health services within the Lewisham Pilot who require a period of re-ablement, stabilisation or social inclusion. They will be working in a senior support planning role undertaking tasks as required as part of providing a high-quality service.
Support Planners will be based within the neighbourhood-based community mental health team which provide integrated health and social care. Their aim will be to provide direct support to service users to reach their full potential, to optimise and maintain their independence, and to exercise choice and control over the way that they live their lives.
South London and Maudsley NHS Foundation Trust (SLaM) offers the widest range of NHS mental health services in the UK, as well as substance misuse services for people addicted to drugs and alcohol. We work closely with the Institute of Psychiatry, Psychology and Neuroscience (IoPPN), King's College London, and are part of King's Health Partners Academic Health Sciences Centre. Our scope is built on three major foundations: care and treatment, science and research, and training.
SLaM employs around 5,000 staff and serves a local population of 1.1 million people. We have more than 230 services, including inpatient wards, outpatient, and community services. Currently, we provide inpatient care for approximately 5,300 people each year and treat more than 45,000 patients in the community across Croydon, Lambeth, Lewisham, and Southwark; as well as substance misuse services for residents of Bexley, Bromley, and Greenwich. Key responsibilities include:
1. Participate in the assessment of service users' mental, social, and physical health needs, including risk assessment and early signs of relapse, and report observations promptly.
2. Work in partnership with service-users, carers, and senior staff to identify goals and develop self-directed support plans.
3. Identify and overcome barriers to achieving goals by providing practical, time-limited support to develop skills and access resources.
4. Plan and deliver support that promotes independence, recovery, and social inclusion, including group interventions.
5. Support care coordinators and occupational therapists in reviewing ongoing care packages post re-ablement, including delivering graded interventions in a person's home.
6. Deliver interventions for a designated caseload within scope of practice.
7. Contribute to regular reviews of progress against service users' goals and revise goals as necessary.
8. Plan and facilitate groups and activities, including risk assessments.
9. Develop relationships with voluntary and non-statutory providers, leading information sharing and updates on community support.
10. Monitor attendance at community groups, clinical outcomes, and initiate new community working methods.
11. Provide cover during staff absences, ensuring duties are completed.
12. Lead the day program, particularly out of hours.
13. Order and monitor stock, ensuring safety standards are met.
14. Assist with the induction of new staff and provide supervision to junior support planners, maintaining records.
15. Address performance issues in conjunction with the Business Manager.
