Job Title: Health and Wellbeing Practitioner
Location: Heathrow Immigration Removal Centre, UK
Salary: £25,154 per annum
Contract: Full time
We are seeking an enthusiastic and proactive Health and Wellbeing Practitioner to join our dynamic team at Heathrow Immigration Removal Centre. Key Responsibilities Attend daily referral meetings and team huddles to support collaborative practice.
Participate in multi-disciplinary reviews, providing informed and constructive input.
Conduct thorough assessments, including risk management and recovery planning.
Manage a caseload and deliver both group and one-to-one interventions.
Promote and signpost relevant services to meet a range of diverse needs.
Plan and coordinate effective discharge and release arrangements.
Maintain detailed and accurate case notes in accordance with data protection and GDPR requirements.
Contribute to service development and wider organisational objectives.
Provide transitional support to individuals preparing for release or deportation, ensuring continuity of care.
Prepare reports and engagement summaries for official and operational use.
Experience, Knowledge, and Qualifications Experience in mental health or substance misuse services.
Proven ability to deliver interventions to adults with complex and varied needs.
Strong IT skills and confidence in developing and implementing SMART care plans.
Good understanding of issues relating to substance misuse and low-level mental health concerns.
Commitment to equality, diversity, and inclusion.
Level 3 Diploma in Health and Social Care (or equivalent). A recognised counselling qualification or similar is highly desirable.
