Woolston and Chartwell Partnership are looking for a Health and Wellbeing Coach to complement the existing Welfare Team. This new role has been put in place to encourage our patients with self-management, motivation levels and supporting them in their commitment to lifestyle change. This can include areas such as diet, exercise, mobility, sleep hygiene, weight management as well as broader mental wellbeing.

You will be part of our growing Welfare Team, working alongside Social Prescribing Link Workers, Mental Health Nurses, Care Coordinators as well as our GPs and clinical teams to develop packages of care suited to the needs of individual patients.

Main duties of the job
This will be a busy and varied role focused on providing personalised support to individuals, their families, and carers to support them to be active participants in their own healthcare as well as empowering them to manage their own health and wellbeing.
Working with GPs and the wider clinical team, you will manage a caseload of patients likely to benefit from health and wellbeing coaching. You will coach and motivate patients through multiple sessions to identify their needs, set goals and achieve their personalised health and care plan objectives.
There will be opportunities to provide group health coaching support for people based on what is important to them, aiming to improve their knowledge, skills, confidence in managing their condition(s).
You will be able to coach individuals across a series of sessions to identify what's important to them; set personal goals and appropriate steps; build skills and confidence to achieve goals; and use problem-solving to work through challenges.
Our health and well being coaches will be able to build trust and respect with the person, providing non-judgemental and non-discriminatory support, respecting diversity and lifestyle choices. They will give our patients the time to tell their stories and focus on what matters to the individuals.
