Overview
Your role is within a creative and friendly Early Intervention team. You’ll have a busy yet fulfilling role assessing adults who are experiencing difficulty with their daily living tasks. You’ll look for ways to improve their independence, wellbeing and quality of life. Through your assessment you’ll recommend support from a range of solutions including a Reablement programme, suitable equipment, adaptations and assistive technology devices. You’ll help people focus on their existing skills, knowledge and experience to achieve positive outcomes. By offering the right initial support you’ll be an important link for the people you’re working with.
Responsibilities
• Assess adults experiencing difficulty with daily living tasks and identify opportunities to improve independence and wellbeing. • Recommend appropriate support options (Reablement programmes, equipment, adaptations, and assistive technology). • Support individuals to build on existing skills, knowledge and experience to achieve positive outcomes. • Act as an important link by delivering initial support and coordinating with other services as needed.
