Patient Flow Coordinator Cornwall Onward Care

We are looking to recruit an enthusiastic Patient Flow Co-ordinator within the Cornwall Onward Care Team, based in Derriford. This person requires previous experience in patient flow.
The aim is to achieve a collaborative approach to hospital discharge, ensuring that patients stay in hospital only for the time required to complete their acute treatment, and that, where necessary, they experience timely and seamless transfer of care to another setting.
To assist with the efficient day-to-day running of the Patient Flow Team, providing administrative and clerical support to Nursing staff, Cornwall Council staff, and other personnel in Derriford, supporting the Onward Care Team.
Main duties of the job
1. Ensure patients' needs are addressed based on the Care plan and coordinated in a timely and effective manner.
2. Act as a liaison, liaising with other agencies and departments to facilitate effective communication and expedite progress towards onward care.
3. Develop working relationships with ward teams to support and coordinate flow across the acute trust.
4. Support the Integrated Transfer of Care Hub in planning and managing the daily flow through the acute trusts, in collaboration with clinical teams, health, and social care agencies.
5. Contribute to follow-up assessments, care planning, resource identification, and service delivery to facilitate timely discharge of patients.

Person Specification
Education/Qualification
* NVQ Level 4 qualification in a relevant subject area or equivalent experience.
* Experience of supervising or managing people.
Skills and Aptitude
* Ability to remain calm in emergencies and respond professionally to stressful and challenging situations.
* Ability to prioritize workload for oneself and others.
Knowledge and Abilities
* Demonstrable knowledge of Patient Flow processes.
* Ability to develop innovative solutions to support patient discharges.
