The role:
As an integral member of the multidisciplinary team, the post-holder will work in partnership, demonstrating compassionate caring behaviours to fulfil the key functions of the CMHT. In particular, the post-holder will be responsible for the assessment of people referred to the service. The post-holder will be involved in the formulation, implementation and evaluation of programmes of care using a strength-based, recovery-focused approach to care that proactively involves the person and optimises self-management. The post-holder is required to be able to undertake duties without direct supervision, working in partnership with patients, carers and other professionals/agencies in line with legislative frameworks, agreed standards and clinical policies. This involves working in a variety of home/near-to-home community environments.
Participating in regular caseload line management supervision and Clinical Supervision are critical aspects of the role.
Supervising the practice of others, undertaking mentorship/preceptorship responsibilities and contributing to the learning and development of the staff team are important aspects of this role.

Key responsibilities:
* Acting as Named Nurse/keyworker within a multidisciplinary team for a defined caseload.
* Undertaking holistic assessment, risk assessment, care planning, implementation and evaluation.
* Delivering appropriate nursing care in the least restrictive manner.
* Promoting mental and physical health and wellbeing.
* Identifying and responding to vulnerability, safeguarding, and risk issues.
* Developing therapeutic relationships with patients and carers, ensuring inclusive, respectful and culturally sensitive care, and working with other agencies to support communication.
* Delivering evidence-based psychological interventions within agreed competencies, individually and in groups.
* Administering and monitoring prescribed treatments in line with policy, including side-effect management.
* Working in partnership with patients and carers to deliver person-centred, values-based care and prevent relapse.
* Ensuring practice complies with the NMC Code and relevant legislation.
* Supporting clinical governance through effective communication, leadership, audit, evidence-based practice, learning, research and risk management.
* Participating in regular supervision, appraisal and PDP processes.
* Engaging in regular clinical supervision and reflective practice.
* Monitoring, supporting and developing the practice of delegated staff.
* Providing line management supervision and appraisal of junior staff and Healthcare Support Workers.
* Supporting learners and student nurses and contributing to a high-quality learning environment.
* Identifying and addressing development needs to ensure safe, effective, person-centred care.
* Liaising with statutory and non-statutory agencies across health and social care settings.
* Maintaining accurate, timely and evidence-based records reflecting risk assessment and care delivery
Knowledge, training, qualifications and/or experience required to do the job:
* Registered Mental Health Nurse - level 1 NMC registration
* Educated to or actively working towards minimum degree level/ or equivalent
* Working knowledge and application of mental health nursing care (including physical healthcare)
* Evidence of active and ongoing CPD
* Evidence of Clinical Supervision/ reflective practice
* Working knowledge of mental health / mental illness/ wellness and recovery
* Working knowledge of rights, values based practice and legislation frameworks
* Evidence of using digital technology/ information systems/ email and basic applications
* Can evidence in career history, the application of a range of skills and behaviours e.g. Organisation skills, Self-directing and Self-motivating, Effective written and verbal communication skills Including negotiation, influencing, persuading, facilitating and conflict resolution skills, Decision making skills
* Ability to work flexibly and to prioritise workload and adapt to meet person centred care without direct supervision
* Ability to form therapeutic professional relationships with others and work as a member of the wider team
* Working knowledge and demonstrable evidence of the NMC Code
* Ability to undertake a number of clinical contacts in a home/ near to home environment on a daily basis to meet which may involve driving
