Physiotherapy Assistant Practitioner

Join our multidisciplinary community team as a Band 4 Therapy Assistant, supporting therapists in delivering high‑quality therapy interventions to patients across the SMASH areas and the CCICP footprint.
Responsibilities
* Make therapeutic contact with newly referred patients as identified by the qualified therapists following admission and triage.
* Independently complete specified holistic assessments, interventions and create rehabilitation programmes either independently or with the help of a qualified clinician for community‑based patients.
* Follow departmental protocols and guidelines and refer to the qualified clinician for advice when necessary.
* Hold a caseload as delegated by the qualified clinician and report back at agreed timescales, highlighting any necessary actions.
* Work independently and support the band 3 Therapy Assistants as required.
* Work with an individual in a variety of settings, such as their home or a 24‑hour care facility, ensuring patient safety and dignity at all times.
* Modify interventions and programmes within defined parameters and within the scope of practice.
* Complete initial assessments and commission a range of basic equipment and adaptations as appropriate to the service user, and perform necessary risk assessments before prescribing basic equipment and reviewing them timely.
* Be responsible for the ordering and stock control of materials and equipment required for the unit.
* Instruct individuals, carers and other professionals in the safe use and maintenance of equipment—simple aids such as walking frames, sticks, elbow crutches and functional aids.
* Refer patients to appropriate services and health‑care professionals where patients are found to have a complex health need.
* Discharge patients within your workload after consultation with a qualified member of the team.
* Attend multidisciplinary meetings and participate in the clinical decision‑making process for individual patients as delegated by the qualified staff.
* Ensure accurate, up‑to‑date clinical records are kept, always ensuring their security and confidentiality.
* Work within your competency and seek qualified therapist guidance where appropriate.
* Participate (pro‑rota) in the 8 am to 8 pm service, 7 days a week to support the Urgent Care Response service when required (not currently practiceul>
Qualifications
o Assistant Practitioner qualification (ST0215 Healthcare Assistant Practitioner Level 5) or equivalent; if not possessed, there is an opportunity to work towards completion.
o BTEC, access to higher education diploma, extensive NHS experience or equivalent.
Skills
o Takes own initiative and seeks appropriate guidance.
o Experience working with people.
o NHS experience as Band 3 or above.
o Experience in working in falls prevention.
Experience
o Experience of contributing to individual & group interventions for patients & settings relevant to the role.
o Experience working within therapy teams.
o Experience of contributing to audit.
