Qualifications
Essential criteria
1. NVQ level 3: health / social care management or equivalent
2. 5 GCSE (to include English & Maths)/or relevant experience
Desirable criteria
3. 1st Level Degree, or working towards
Experience
Essential criteria
4. Experience of working with patients with liver disease
5. Experience of or working in healthcare settings, hospital or community or joint working with healthcare services
6. Experience of face to face communication with patients
Desirable criteria
7. Trained and experienced in fibroscanning.
8. Experience of working with vulnerable adults in a care coordinating, supporting, advisory or advocacy role.
Technical Skills Competencies
Essential criteria
9. Ability to document care assessments/progress
10. Use of IT including Windows, Word, Excel, PowerPoint
Desirable criteria
11. Experience of accessing hospital IT systems, eCamis, EPR
Knowledge
Essential criteria
12. Knowledge of assessing and problem solving issues related to care planning
Interpersonal skills
Essential criteria
13. Excellent assessment, planning, care coordination and organisational skills
14. Excellent oral and written communication skills, ability to engage with and manage expectations of patients with complex needs, and complex issues in a way that is appropriate for the audience
