Overview
We are seeking a passionate and motivated Band 6 Social Worker to join a dedicated multidisciplinary team at Parklands Hospital on a 3‑month temporary contract. This is a full‑time position. The role is an exciting opportunity for a qualified Social Worker to play a vital role in supporting adults receiving inpatient mental health care, ensuring timely, safe, and person‑centred discharge planning from admission through to recovery.
Job Responsibilities
* Leading and coordinating discharge planning from the point of admission.
* Identifying and addressing barriers to discharge to prevent delays.
* Supporting Care Act assessments and access to appropriate care funding.
* Working collaboratively within a multidisciplinary team to assess, plan, and review care.
* Providing specialist social work input regarding safeguarding adults and children.
* Advising colleagues on the Mental Capacity Act and relevant legal frameworks.
* Building strong partnerships with families, carers, local authorities, housing providers, and community services.
* Managing clinical risk and contributing to high‑quality patient care.
* Maintaining accurate electronic records and professional documentation.
* Acting as a safeguarding lead within the inpatient team.
Qualifications
* Degree in Social Work (or equivalent professional qualification).
* Current registration with Social Work England.
* Experience working with adults with mental health needs.
* Knowledge of the Care Act, Mental Health Act, Mental Capacity Act, and safeguarding processes.
* Ability to manage a varied workload and work independently.
