We are seeking highly motivated and enthusiastic additional PCN pharmacists tojoin our well-established team to work on projects that will containa split of PCN objectives such as SMRs, delivered in closepartnership with a registered practice, and practice support, mainlyconcerning meds management, delivered in a developing hub model.

This is an exciting opportunity to develop anew patient-facing role within a PCN and will develop over time and thepotential to develop special interests in line with the PCN objectives andpriorities. Full clinical and administrative support will be provided, andwhilst expected to work autonomously you will also be a valuablemember of a highly skilled and committed multi-disciplinary team.

The successful candidate will be based within the locality either at a hub or within a member GP practice (not necessarily in Whaley Bridge) with the potential for some home working where appropriate.


Main duties of the job

Keen to work part of a multi-disciplinary team in a patient-facing role

Able to conduct structured medication reviews to proactively manage patients with complex polypharmacy

Able to provide clinical medicines expertise to both patients and general practice team

Keen to improve prescribing practices across the Primary Care Network

Keen to improve service and quality through mechanisms such as audits and PDSA cycles

Able to interface with community and hospital colleagues to help resolve medication-related issues

Interested in developing specific areas of interest and expertise within the needs of the locality as a whole.


About us

High Peak PCN was formed in July 2019 and consists of 8 high quality GP practices rated Good or Outstanding by the CQC. The practices have a history of positively and constructively working together to improve services for their patient populations. The practices serve the areas of the North High Peak, Buxton and Hartington, with good transport links in to Stockport, Manchester, Chesterfield and Sheffield.

Within the PCN umbrella we have a successful and well established team of Pharmacists and Social Prescribers, as well as CPNs, End of Life and Dementia Care Co-Ordinator's and physios. We are looking to further develop additional roles over the coming months.

Person Specification


Qualifications

* Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council
* Member of the Royal Pharmaceutical Society (RPS)
* Experience of medication review
* Proven clinical pharmacy knowledge and evidence of commitment to continuing education
* Understanding of current medicines management issues
* Member of or working towards (through foundation)
* RPS Faculty membership
* Hold or be working towards a prescribing qualification
* Hold or be working towards a postgraduate pharmacy qualification
* Have completed the CPPE pathway for PCN Pharmacists


Experience

* Minimum of 2 years post graduate experience in pharmacy, as demonstrated within a practice portfolio
* Has an awareness of the breadth of common acute and long term conditions that are likely to be seen in a general medical practice
* Experience of working in multi-disciplinary teams
* Understanding of primary and secondary care interface
* Familiarity with the concepts of evidence based medicine, needs assessment, effectiveness, prioritisation and appropriateness
* An appreciation of the complexities of relationships across the health economy.
* Experience of working within a primary care setting
* 3 years post-registration pharmaceutical experience including older peoples care
* Knowledge of the changing NHS environment and structures
* Experience of service provision, advice and training to care homes


Non-Clinical Skills & Personal Qualities

* Good organisational skill
* Good time management
* Good numeracy and analytical skills Confident and assertive in dealing with other professionals
* Ability to use initiative
* Self-motivated
* Good presentation and teaching skills
* Ability to work independently or as part of a multi-disciplinary team
* Ability to influence change
* Good computer skills including working knowledge of Microsoft office
* Comprehensive understanding of and commitment to the application of GPDR, Information Governance and Confidentiality requirements when accessing sensitive data
* Excellent interpersonal skills particularly with prescribers, older people and carers
* Ability to deal with difficult issues/situations with diplomacy and sensitivity
* Ability to work on own initiative, able to plan, prioritise, make decisions and meet deadlines
* Ability to work well in a multi-disciplinary team
* Current driving licence and access to own transport
* Knowledge of GP computer systems especially EMIS Web
* Familiarity with analysing/interpreting ePACT data
