Community Support Worker (Discharge to Assess Team) – Permanent, Full Time. Location: Basildon. Working Style: Community-based worker. Salary: £25,395 to £32,131 per annum. Closing Date: 9th October 2025.
We have an exciting opportunity within Early Intervention in Southwest Essex for a Community Support Worker. You’ll be working within the Discharge to Assess Team, based in Basildon, covering all areas of Basildon. If you’re caring, driven, innovative, and eager to make an impact, we want to hear from you!
The team works on a 6-week rolling rota which includes working 1 weekend every 6 weeks and 1 bank holiday each year. There is a 6% allowance given as compensation. As an ‘Community Based Worker’ the role will involve a combination of remote, community and office based working. There is an expectation to attend the office at Ely House, Basildon, approximately 2-3 days per week in addition to ‘as and when’ there is a business need. The role requires mobility across the operational area; therefore, a driving licence and access to a vehicle are required, or the ability to meet mobility requirements by other means.
The Role: As a Community Support Worker, you will undertake assessments and reviews in line with statutory duties under the Care Act to identify eligible needs and generate a person-centred and outcome-focused support plan. You will work in partnership with internal and external colleagues/partners to achieve the best outcomes for Adults and Carers. This is a community-based role with regular home visits, meetings with colleagues, and time spent in the office and/or working from home. You will work with adults of all ages (18+), in a supportive team that encourages career development. An induction and appropriate training will be provided.
