As part of the Rotherham Healthwave team, our Health and Wellbeing Coaches support people from the local community to improve how they manage their physical and mental health. By working with individuals both one-on-one and in groups, you will help develop their knowledge, skills, and confidence to change health-related behaviors, thereby improving overall quality of life, health, and wellbeing. These behaviors include dietary advice, smoking cessation, and physical activity.
As a Health and Wellbeing Coach, you will be responsible for maintaining accurate clinical records. The role requires coaching skills, patience, and empathy to support clients in achieving their goals. A person-centered approach is essential for the success of the service. While the role can be challenging, it is also highly rewarding.

Connnnect Healthcare Rotherham Community Interest Company (CIC) is a federation of 28 GP practices in Rotherham, led by a Board of practice representatives and a part-time Medical Director. The organization delivers extended access primary care services and a Physio First service in partnership with local trusts. It supports primary care networks and provides training to the general practice workforce.

Key Responsibilities
1. Manage and maintain a caseload of clients, developing tailored support plans based on assessed needs, including interventions, signposting, and referrals.
2. Work with clients to set SMART goals following NICE and NHS guidelines.
3. Deliver nutrition and weight management sessions, including taking weights and waist measurements confidentially.
4. Contribute to service monitoring and evaluation, including data collection and follow-up with non-engaged clients.
5. Accurately document patient information in electronic health records.
6. Collaborate with managers and IT support to ensure data quality and support achievement of targets.
7. Manage personal time and priorities effectively.
8. Undertake additional tasks as assigned by managers and colleagues.
9. Maintain confidentiality in line with organizational policies.
10. Support team practices and work flexibly with colleagues.
11. Participate in community and promotional events.
12. Communicate work capacity and pressures effectively to ensure smooth administrative operations.
13. Attend mandatory training and comply with health and safety regulations.
14. Design, upload, and staff rotas for the Weight Management and Nutrition team.
15. Collaborate with the wider team to deliver Rotherham Healthwave services.
