The post is a permanent contract, full time at 37hrs per week.

Main duties of the job
As a Social Worker in our Learning Disability team, you will undertake care and support needs assessments, care planning and reviews, complete Care Act and Mental Capacity assessments, and work closely with individuals, families, and carers to create tailored care plans that truly make a difference. Youll also collaborate with a range of other internal and external statutory, private and charitable and voluntary stakeholders to ensure our residents receive the right support and resources to achieve better outcomes.
Our Learning Disability team is co-located with specialist health partners, fostering strong collaborative relationships that lead to more integrated working and better outcomes for the individuals and families we support.
Youll enjoy autonomy in managing your caseload working closely with a supportive team while receiving consistent guidance from Advanced Social Work Practitioners and your Team Manager.

* Experience of Social Work with adults you will have experience of working within Adult Social Care teams in particular with clients whose primary need is a learning disability.
* It is an essential requirement of the role that the post holder has a valid UK driving licence
