About the Role Based across Arrowe Park Hospital and Clatterbridge offices, you will work within a dynamic multi-disciplinary team, supporting safe, timely discharges from hospital while promoting independence, wellbeing, and positive outcomes. As an Experienced Social Worker, you will contribute to the delivery of high‑quality health and social care functions for adults and carers, ensuring our practice reflects the Care Act 2014, strength‑based working, personalisation, and social inclusion. You will work with adults with physical health needs, mental health needs, older adults, and carers—often those experiencing complex circumstances. This role also includes supporting the development of others, including ASYE/newly qualified social workers and Care Navigators, modelling good practice and championing professional standards. About the Candidate We are looking for someone with a strong background, working with adults with complex needs, carers and families. They must be skilled in assessments, support planning, safeguarding, and working within statutory frameworks. Excellent communication skills will be required, both written and verbal, with the ability to manage complex and sensitive situations including prioritising demanding workloads and maintaining high-quality practice. Social Work qualification required (e.g. Degree in Social Work, DipSW or equivalent) and registered with Social Work England.
