Job overview

An exciting opportunity has become available to join our small and friendly team. The Chlamydia Screening, Home Sampling and Health Promotion Practitioner role sits within the Chlamydia Screening Team, which delivers the National Chlamydia Screening Programme across Brighton & Hove; the at-home STI testing service, and C-Card Scheme. The team is embedded in the Sexual Health and Contraception Service (SHAC), and is based at the Royal Sussex County Hospital. The Chlamydia Screening, Home Sampling and Health Promotion Practitioner role supports the Health Promotion Team deliver the Sexual Health Promotion aims for under 25-year-olds across the city through education, outreach and community collaboration, including a targeted approach to engaging with hard-to-reach groups and improvement to access of SHAC services. The role is also responsible for providing administrative support to the wider Chlamydia Screening Team, which includes responding to telephone enquiries; logging and giving results; maintenance of electronic patient records; ordering and management of stock, and support of the C-Card free condom scheme. The Chlamydia Screening Team promote a culture of inclusivity and support, and we are looking for someone with a passion for Sexual Health and working with young people to join our dynamic team.

Main duties of the job

The main duties of this role will include administration of the at-home STI testing service, the local under 25's Chlamydia Screening Programme, and the Brighton and Hove C-Card free condom scheme. The post-holder will also work as part of the health promotion team to develop and deliver sexual health promotion initiatives, and support in the delivery of outreach and education sessions including to under 25s. The post holder will oversee administrative staff and patient management processes.

Detailed job description and main responsibilities

·To contribute to the community treatment pathway as contact person for community treatment providers, ensuring they are clear on and adhere to processes and monitor with data

·Be a contact person for pharmacies who provide free emergency contraception (EHC). Monitor EHC data and use data to inform CSP practice to increase uptake of CSP kits for <25’s accessing this service

·Manage daily positive Chlamydia and Gonorrhoea results list and its associated interpretation and administration which will involve recalling patients diagnosed with Chlamydia and Gonorrhoea that require treatment, the follow up of patients who have had treatment to check compliance and partner notification completion and where appropriate referring to the health adviser team for further information or support

·To be responsible for overseeing negative text message results and ensuring processes run smoothly

·To lead on administration support to the Health Promotion Specialists, overseeing administrators to ensure resources are ready for session and training delivery

·To work as an effective and responsive team member, arriving promptly and working effectively during agreed hours

·To provide diagnosis and information to patients over the telephone and to deal with the complexities that may subsequently arise from these in a sensitive and empathetic manner

·To make sound decisions around recalling patients and ensure the best course of action in facilitating the prompt treatment of patients

·To be responsible for documenting patient care on EPR and clearly recording actions taken on a daily basis

·To take a lead role in the follow up of partner notification conversations with patients in the recall system and to use SXT digital partner notification tool as necessary

Essential criteria

1. •Higher National Certificate/Vocational Qualification at Level 4 or 5/Foundation Degree/Higher
2. •IT Skills to RSA3/NVQ Level 3, or equivalent experience
3. •Maths and English to GCSE or equivalent
4. •Proven ability to manage and analyse data
5. •Competent in a range of packages including MS Word and MS Excel, and willing to learn new packages and advance current skills

6. •Evidence of having undertaken own development to improve understanding of equalities issues
7. •Experience of working in a paid or voluntary capacity with residents/community groups
8. •Excellent numerical, verbal and written English skills
9. •Understanding of risk taking in the Under 25s including sexual health and drugs and alcohol
10. •Experience of delivering training and presentations to community groups
11. •Knowledge of Brighton & Hove education settings, community groups and services working with the Under 25s
12. •Experience of working with young people in a variety of settings including vulnerable groups
13. Experience in measurement and evaluation of current practices through the use of Evidence Based Practice projects, audit, research, outcome measures and make recommendations for change based on the findings
14. Experience in managing invoices, requesting payments and ordering stock
15. •Ability to develop standards for use within the department, guidance, policies and procedures relating to management of the CSP and home-sampling
16. •Able to build positive working relationships with a range of professionals and stakeholders

23. •Familiar with or willingness to gain confidence with novel technological ways to deliver information

25. •Able to demonstrate advanced problem-solving skills, developing alternative ways of working to maximise team efficiency
26. •Ability to present, interpret and compare data relevant to the Chlamydia Screening Programme and to note trends and where necessary take appropriate action by escalating/referring to appropriate person
27. •Ability to independently assess situations and to identify necessary resources to solve problems
28. •Proven track record demonstrating ability to make timely and effective decisions
29. •Works autonomously and follows standard SHAC Service and Trust procedures and practices. Works to meet targets and deadlines as agreed with line manager
