Overview
Diabetes Specialist Nurse – 22.2 Hours Per Week Permanent (3 days a week Monday‑Friday). This role works across the hospital and community, collaborating closely with the multidisciplinary team to ensure patients see the right person at the right time in the right place.
Responsibilities
* Provide specialist diabetes care to patients in hospital and community settings.
* Work seamlessly with the multidisciplinary team to coordinate care and support patient self‑management.
* Ensure patients receive timely and appropriate interventions to manage their diabetes.
Qualifications
* Highly motivated with a keen interest in diabetes.
* Previous experience within the service is desirable but not essential.
