B7 Nurse Practitioner in our Memory and Dementia Pathway based at HMP Dove g ate, Staffordshire
An opportunity has arisen to recruit a band 7 mental health Nurse Practitioner within the Memory and Pathway based within the prison environment, providing specialist diagnostic assessment for dementia and following this up with specialist treatment and monitoring, alongside existing care services within the prison.
Applicants should possess the knowledge and skills to enable them to work in the memory and dementia speciality, working with service users affected by all types of dementia. This includes working with challenging and complex needs associated to a dementia diagnosis. Applicants should have non-medical prescribing qualification.
Applicants will be part of the South Staffordshire Memory and Dementia service but working clinically in a designated prison environment s (management and clinical supervision will provided via the core Memory & Dementia pathway).
Applicants will be responsible for the delivery of a range of high-level specialist assessments, identifying risks and management plans in the process.
Delivery of a high standard of nursing care to individuals referred to the Memory & Dementia Service ( HMP Dove Gate )
Provide a specialist mental health nursing assessment, care planning and to evaluate those experiencing dementia
To work in partnership with fellow team members and internal/external partners.
To undertake their own professional development and compliance with mandatory training.
