Job Purpose
* To assess the accommodation needs of individuals and families, ensuring that appropriate housing solutions are identified and implemented in line with statutory obligations and local policies. The role involves working closely with vulnerable clients, partner agencies, and housing providers to deliver tailored support and sustainable housing outcomes.
Key Duties/Accountabilities
* Conduct comprehensive assessments of clients’ housing and support needs.
* Develop and implement personalised housing plans in collaboration with clients.
* Liaise with internal departments, local authorities, and external agencies to coordinate support services.
* Provide advice and guidance on housing options, welfare benefits, and tenancy sustainment.
* Maintain accurate and up-to-date case records and documentation.
Essential Experience Required
* Proven experience in housing, social care, or a related field.
* Experience conducting needs assessments and developing support plans.
Essential Qualification Required
* A relevant qualification in housing, social work, health and social care, or equivalent (e.g., CIH Level 3 or above).
Additional information to note
* Working hours: 36 hours per week
