Service Lead - Community Transfer of Care Hubs
We are seeking an experienced and motivated Service Lead to provide strategic leadership and operational management for our Community Transfer of Care Hubs (CToCH). This is a high-profile leadership role within a developing, system-facing service that plays a critical part in supporting safe, timely discharge from acute hospital settings.
You will be responsible for the ongoing development, quality, and performance of the service, ensuring delivery is person-centred, equitable, and aligned with organisational priorities, integrated care agendas, and national NHS policy.

Service Overview
The CToCH service has evolved over recent years from three geographically based teams into a single, integrated hub model, improving equity, efficiency, and system-wide collaboration. The service works closely with acute trusts, community services, local authorities, and voluntary sector partners to support effective transitions of care and improved patient outcomes.
Main duties of the job
The Service Lead will:
* Provide visible, inclusive, and strategic leadership to the CToCH service, promoting a culture of excellence, continuous improvement, and compassionate leadership.
* Be accountable for operational performance, quality, safety, and financial stewardship within the service.
* Lead and support Operational and Administrative Leads, empowering Case Managers and support staff to deliver consistently high standards of care.
* Ensure services are delivered in accordance with NHS standards, clinical governance frameworks, and best practice.
* Drive service improvement and transformation, using data, insight, and innovation, including digital solutions, to improve patient flow and experience.
* Work collaboratively with internal and external stakeholders to co-produce pathways and processes, strengthening system integration and partnership working.
* Lead and manage a multidisciplinary workforce in an integrated, efficient, and cost-effective way.
* Contribute to wider organisational objectives and represent the service at relevant operational and strategic forums.

Person Specification
Qualifications
* Registered health care professional (Nurse or Allied Health Professional) with current professional registration.
* Recognised postgraduate management or leadership qualification, or willingness to work towards one.
Experience
* Significant post-registration experience within a health and/or social care community setting or comparable multidisciplinary environment.
* Proven experience of operational and service leadership, including managing and developing multidisciplinary teams across multiple sites.
* Demonstrable experience of leading through change and supporting staff within a transforming service.
* Experience of managing complex service issues, including staff-related matters and service pressures.
* Experience of managing service performance and quality improvement activity.
* Experience of working collaboratively with internal and external stakeholders to deliver integrated services.
Knowledge
* Excellent problem-solving and decision-making skills, with the ability to consider organisational and system-wide impacts.
* Ability to understand and manage complex situations, often with incomplete information, and develop effective solutions.
* Highly developed communication skills, including the ability to present and explain complex information to a range of audiences.
