Responsibilities
* Support the team from a clinical and leadership perspective in a service operating 24 hours a day, 7 days a week.
* Ensure overall standards including assessment of need, risk screening and delivery of care in line with organisational policy and procedures.
* Maintain continuity of care for service users throughout their journey through the service.
* Be multi‑skilled, adaptable and flexible in delivering care within a variety of settings.
* Provide triage and assessments, delivering enhanced risk assessment and care to those presenting in mental health crisis.
* Work closely with the Clinical / Operational Leadership Team to ensure effective and safe services.
* Provide sound clinical supervision to junior staff and effective communication within the team.
* Provide education within the team and wider health economy in line with Key Performance Targets.
* Support the development of a skilled and flexible workforce within the MH Triage and Response and Mental Health Crisis Assessment Service (MHCAS), ensuring staff maintain appropriate skills.
* Carry out assessments, evaluation and safety planning for clients and engage in safety huddles with the wider team.
* Develop care plans that meet the assessed need of each individual and ensure unqualified staff deliver care as identified in the care plan.
* Liaise with family members, carers and significant others when completing assessments and developing care plans.
* Supervise risk assessments and formulate risk management plans for service users under your care.
* Ensure Safeguarding Children and Adults requirements are followed and participate in meetings as appropriate.
* Support unqualified staff to ensure delivery of high standard of care to service users.
* Participate in setting standards for care based on sound evidence‑based and research findings.
* Communicate with other multidisciplinary team members information that impacts the service user's care.
* Use verbal and non‑verbal communication skills to impart sensitive information.
* Check understanding of information where there are barriers to effective communication.
* Provide accurate up‑to‑date information regarding service users' care in reviews and multi‑disciplinary meetings.
* Maintain accurate knowledge of the Mental Health Act / Mental Capacity Act and other relevant legislation.
* Document all information about a service user's care and treatment accurately and legibly in the health record; supervise and monitor entries made by unqualified staff.
* Document all incidents or near‑misses as per trust policy and take any required follow‑up action.
* React proactively in cases of service user crisis and put in place immediate plans to resolve the situation with appropriate supervision and guidance.
* Actively participate in team meetings, development and reflective practice.
* Maintain relevant professional registration and PREP requirements.
* Engage in ongoing learning to maintain and develop practice utilising CPD, self‑directed learning, reflective practice, coaching, mentorship, clinical supervision and performance management.
* Serve as a link between mental health services and primary care in an advisory capacity.
* Develop effective working relationships with professionals outside the Trust to enhance delivery of care.
* Supervise and be supervised within the Divisional performance management / supervision framework.
* Attend, as appropriate, courses and conferences and share knowledge gained from attendance.
* Act in accordance with trust policies and procedures, ensuring unqualified staff are adhering to policy.
* Undertake mandatory training as identified by the Trust and Division.
* Keep up to date with developments in professional practice and recommend changes to senior staff.
Qualifications & Requirements
* Valid mental health clinical registration.
* Relevant professional registration and ongoing PREP compliance.
* Knowledge of the Mental Health Act, Mental Capacity Act and other relevant legislation.
* Level 2 Police vetting is required for working alongside Police/BTP.
* Enrolment in the DBS Update Service (enhanced DBS) required for Children & Young People services; fee £16 per year.
* Certifications for disability confidence and guaranteed interview scheme are available for disabled applicants meeting essential criteria.

The Mental Health Triage and Response team is a dynamic new service which incorporates the Multiple Award winning Street Triage Car, Health Based Places of Safety (HBPoS) and the new Mental Health Crisis Assessment Service (MHCAS) that fall within the Mersey Care Footprint. It plays an important role in Mersey Care's response to the national roll out of Right Care Right Person across the region.
