Job summary Providing leadership to the Neighbourhood Care Network (NCN) to ensure the development and provision of high-quality integrated health and social care services through Place Based Care, for the people who are registered for general practice care within the Newport West Network boundary area. To support the development and implementation of strategic and operational plans for the Division, particularly aligning to Place Based Care. To provide leadership and to secure engagement from Primary, Community and Secondary Care for neighbourhood service planning. Working closely with the Locality Management Team and other officers of the Health Board to ensure the delivery of the objectives set out for the Neighbourhood Care Networks in respective strategies. To support the delivery of high quality and equitable primary care services across the neighbourhood By working in collaboration with the other NCN leads and as part of the wider Health Board clinical management team to provide clinical leadership, motivation and development through GMS Collaboratives and collectively to drive forward service transformation at an individual practice level reflective of the population need. To work with and support practices in undertaking of Quality Improvement projects, drawing upon Health Board resources where appropriate. To inform and engage where appropriate with the Clinical Interface Group Main duties of the job Leadership o Provide leadership and to secure engagement from Primary and Secondary Care for neighbourhood service planning. o Lead/chair localised NCN meetings provide holistic development and support across contracted services and professions and contribute to Health Board wide meetings where appropriate. Quality Improvement & Delivering Excellent Clinical Services o Support the delivery of high quality and equitable core primary care and community services across the neighbourhood. o Driving improvements and innovation in the delivery of services to ensure that services are continuously improved by reducing waste, harm and variation and supporting the adoption of locally agreed and evidence-based care pathways. Patient Quality and Safety o Assist in assuring appropriate clinical governance arrangements across the NCN. o Support the Primary Care Clinical Directors and Divisional Nurse in developing a culture where Patient Safety and Quality have the highest priority. o Drive clinical quality improvements across all aspects of primary and community care in the network.
