Job summary
Cardiac Intensive Care/C604 is located at the Bristol Heart Institute, Level 6. We care for all patients post cardiac surgery both emergency and planned, as well as some planned general surgery patients.
Our patient's include all patients having cardiac surgery, adult congenital heart disease and some patients having cardiology procedures, as well as some planned general surgery procedures. Our patient length of stay is from 2-7 days, but can sometimes be longer.
The ward has a Multi-disciplinary approach where you will interact regularly Physiotherapist, Occupational Therapist, Surgeons, Nurse Practitioners and Specialist Nurses like heart failure Nurses or wound care Nurses along with the Nurses and Doctors' caring for our patients.
Main duties of the job
1. To liaise with patients, clinical staff, GPs and Trust support staff in person, via telephone and email as and when necessary
2. Show a caring and professional approach towards patients and visitors when carrying out all duties
3. Escalate patient queries, capacity problems and general issues to the role's line manager or relevant clinical staff as appropriate
4. To welcome and assist patients, relatives and visitors on arrival to the ward and inform appropriate staff. This may require acting with resilience to deal with sometimes distressed patients and relatives in a professional and empathetic manner
5. Ensure that all clinical results and other clinical information are accurately filed in the patient notes and that patient notes are up to date and maintained according to the relevant Trust standards
6. Ensure patients are transferred to relevant wards on care flow (hospital tracking system)
7. Ensure patient records are appropriately received and transferred and trace all notes into and out of the ward
8. Prepare patient notes for coding within the relevant deadlines set by the coding team
9. To carry out associated clerical duties, for example maintaining accurate filing systems, photocopying, receiving and distributing post, diary management and scanning/uploading patient notes to the online computer system so that they are ready to be viewed in their out patient appt, making outpatient appointments
10. Undertake the role's/duties in the case of a major incident in your department
