We’re looking for two Social Care Practitioners to join our well-established Shared Lives Team for 12 months.
Shared Lives Social Care Practitioners are a key part of the team and if successful you will be:
1. Working with your own caseload of Shared Lives carers to ensure they meet the requirements of the scheme, relevant legislation and CQC guidelines.
2. Providing support to Shared Lives carers, carrying out home visits, finding solutions to challenges that may arise and undertaking annual Shared Lives carer reviews.
3. Participating fully in the matching process.
4. Producing high quality personal service plans, risk assessments and risk enablement plans.
5. Promoting Shared Lives through internal and external networking and promotional events.
You must be an organised person who can manage their time well, an excellent communicator who can use their inter-personal skills to develop and maintain effective relationship with our Shared Lives carers and other members of the Team.
The Team is small but very supportive, with a unique position across both social work and direct care services. In joining the Team, you would receive a full induction, regular supervision, opportunities to develop your practice and see first-hand the difference Shared Lives can make.
The Team covers the whole of Derbyshire with a team base in Amber Valley, and works in a hybrid way, working from home, and local offices.
As these posts are fixed term for 12 months, we would also welcome applications from people wishing to undertake them on a secondment basis.
