An exciting opportunity has arisen for a dynamic Occupational Therapist to join our Specialist Palliative Care (SPC) team.

We are seeking a positive and compassionate individual with a strong commitment to delivering excellent patient care. The successful candidate will work closely with individuals facing life-limiting conditions, supporting them to maintain dignity, independence, and quality of life through meaningful activity and personalised interventions.
The Therapy team delivers clinically effective, person-centred care across multiple sites within CCICP, serving the communities of South Cheshire and Vale Royal. The team includes Occupational Therapists, Physiotherapists, Dietitians, and Therapy Assistants, and works closely with Community SPC Nurses, Medics, and St Luke’s Hospice as part of the wider Specialist Palliative Care network.
We are ideally looking for an established Band 7 Occupational Therapist with specialist palliative care experience. However, we welcome applications from Band 6 Occupational Therapists with extensive experience in community settings or managing complex long-term conditions, for consideration as a Band 6–7 development role.
* Deliver holistic assessments and tailored interventions to support patients’ independence and dignity
* Collaborate with the multidisciplinary team to ensure coordinated care
* Identify opportunities for service improvement and contribute to quality initiatives
* Support the development of resources, pathways, and best practices
* Provide education and guidance to patients, carers, and colleagues
* Participate in audits and reflective practice to enhance service delivery
Mid Cheshire Hospitals NHS Foundation Trust provides comprehensive hospital and community services for East Cheshire (population 399K) and West Cheshire and Chester (population 357K). Our services include planned and unplanned/emergency care, cardiac and critical care, child health, maternity, and intermediate care across Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford.
Through our community arm, Central Cheshire Integrated Care Partnership, we deliver extensive services in 26 medical centres and schools in collaboration with Cheshire and Wirral Partnership NHS FT and the South Cheshire and Vale Royal GP Alliance.
