Role Summary
* To ensure that both urgent and non-urgent referrals to Acute Medicine Ambulatory are managed in a timely way. This includes working with clinicians to screen referrals and schedule patients appropriately. This will involve contacting patients and balancing meeting their needs within the constraints of the service.
* To maintain effective and efficient coordination of our Virtual Ward activity.
* To provide administrative support to the two Clinical Nurse Managers as needed.
* To manage the digital devices utilised by the Acute Hospital at Home team for remote management of patients. This includes monitoring stock, tracking equipment, wiping and rebuilding smart phones and smart watches.
* Provide excellent customer care which may include communication with distressed and anxious patients and relatives, treating them with tact and empathy.
* Ensure the professional image of the Trust is maintained at all times.
* To manage the band 3 Administrators across our North and Eastern sites

This is an exciting development opportunity for an experienced administrator to bring their experience and knowledge to our integrated Virtual Ward Team as an Acute Medicine Care Coordinator. This role is based in the RD&E Hospital in Exeter, but will involve working in NDDH in Barnstaple when required. You will be managed by the Eastern AHAH Clinical Nurse Manager.
Virtual ward is a safe and efficient alternative to NHS bedded care. Virtual wards support patients who would otherwise be in hospital to receive the acute care and treatment they need in their own home. This includes either preventing avoidable admissions into hospital or supporting early discharge out of hospital.
The delivery of digitally enabled care (virtual care) including virtual wards is a national priority embedded within current health and social care policy. Our Virtual Ward has 100 beds at present, split across Northern, Eastern and Central localities.
This post, as part of a multi-disciplinary team, provides an opportunity to drive and transform the system by embedding the use of digital technology to release pressure on acute care and improve patient outcomes.
You will monitor and ensure the management of patients is efficient and effective, considering the individual needs of the patient and the requirements of the service. A key part of the role will be providing administrative support to all clinical and non-clinical colleagues involved, link in with community teams and other services as requested.
As we move towards a new era of integrated care and working collaboratively across organisational boundaries, the need for dynamic, focused teamwork is vital.
Working Pattern:15 hours per week. Two 7.5 hour days per week, Monday and Tuesday.
