Role Responsibility

We are offering an excellent opportunity to join a friendly, dynamic, and supportive team which promotes professional development.

The post would be based in the West Kent South area and focusing on the work for discharge planning for clients under the Older Persons Physical Disability (OPPD) remit. This post is within a team that works collaboratively with health and other partnership agencies and focuses on promoting a persons independence.

We are looking for motivated, experienced professionals who are interested in working within the Short-Term Pathways, and keen to work with Acute, Community Hospitals. The successful applicant also needs to be flexible in their approach and committed to working in an integrated team and ready to embrace new ways of working, to help further shape our services.

As a senior practitioner within the Short-Term Pathways team, you will be providing practice and supervisory service to a designated group of professional staff in the Short-Term Pathways team, ensuring effective safe practice, high quality case work / service and performance in the designated team through the application of professional expertise to the team and wider teams. Ensure adherence to the operating framework for OP/PD clients and related policies, procedures, and practice guidance.

Management of a client caseload made up of highly complex / safeguarding cases that require a high level of specialist expertise to ensure that a proportionate assessment is undertaken to identify required needs, risks, and outcomes, to promote enablement and ensure an appropriate level of resource allocation is made to meet peoples support needs.

The ability to work closely with other social and health colleagues, including the teams Occupational Therapists to promote independence throughout recovery.

Our team in Short Term Pathways is creative, energetic, dynamic, and diverse. The team are proud of the high standard of service we provide to our customers at their most vulnerable times.

We are increasingly accessible to the public outside of traditional office hours, as we move towards an 8 am to 8 pm, 7 days a week service to ensure service needs are met.

This post is considered by KCC to be a customer-facing position. The Council
therefore has a statutory duty under Part 7 of the Immigration Act (2016) to
ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.

This post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and registration as a qualified Social Worker with Social Work England.

This role is subject to holding a Full UK Driving Licence The Council is committed to making reasonable adjustments so whilst this job requires the jobholder to drive your application will still be considered if you are unable to drive due to a disability.
