Job overview Surrey and Borders is looking for someone who is passionate about ensuring a high-quality holistic approach to the care and support provided to clients in the community who have been identified as having cognitive/memory problems. Work Location: Gatton Place, St Matthews Road, Redhill, RH1 1TA Working Pattern: Monday to Friday from 9 AM to 5 PM Advert Deliver high-quality community mental health nursing services to older adults within a multi-professional framework. Undertake autonomous assessment, nurse diagnosis, care planning, and treatment delivery. Discharge professional responsibilities under the Mental Health Act (1983), Mental Capacity Act (2005), and the NMC Code of Conduct. Manage and coordinate care for a defined caseload under the Care Programme Approach (CPA). Administer and manage medications safely in accordance with Trust and national guidelines. Maintain accurate clinical records in line with Caldicott principles and Trust standards. Detailed job description and main responsibilities Professional and Legal Responsibilities Practice in accordance with the Mental Health Act (1983), Mental Capacity Act (2005), and the NMC Code of Conduct, acting as a professional role model at all times. Adhere to Trust policies, lone working procedures, and safeguarding responsibilities. Maintain confidentiality and ensure documentation complies with Caldicott principles and Trust standards. Clinical Responsibilities Provide comprehensive assessment, formulation, nurse diagnosis, and evidence-based interventions for older people with mental health and cognitive problems in community settings. Deliver care autonomously within a multi-disciplinary framework, including the ability to lone work safely. Act as Key Worker/Lead Professional under the Care Programme Approach, including completion of Single Assessments. Develop, implement, and regularly review person-centred care plans based on assessed need. Refer to appropriate professional services when required to ensure holistic care delivery Medication Management Ensure safe storage, administration, and recording of medicines within clinical and community settings. Administer intramuscular depot injections in line with Trust and national guidance. Support the safe delivery and monitoring of oral medications within service users’ homes. Caseload and Supervision Hold continuous responsibility for the management of a defined caseload. Receive clinical and management supervision in accordance with Band 6 expectations. Attending to patients in the clinic or in their own homes for the purpose of undertaking cognitive/memory assessments Governance, Risk and Quality Ensure timely reporting of incidents, complaints, and risks in accordance with Trust procedures. Inform the Line Manager/Team Leader of any incidents affecting service delivery. Cooperate fully with investigations and contribute to learning from incidents. Maintain up-to-date, high-quality records using the Trust clinical record system and other IT systems. Education, Training and Development Develop and maintain a personal learning plan in agreement with supervisor to ensure continued professional development. Deliver education and training to junior staff, student nurses, and other professionals regarding community mental health nursing competencies. Act as a health advisor and educator across primary and secondary care settings, promoting positive approaches to physical and mental health for service users, carers, and professionals.
