Join to apply for the Community Psychiatric Nurse role at NHS
The CRHT is a multidisciplinary (MDT) team that provides 24/7 urgent clinical triage and advice via the crisis telephone line, and conducts urgent and emergency mental health assessments for people in crisis. The team also provides intensive home treatment as an alternative to hospital admission and acts as gatekeepers for inpatient admissions, crisis house admissions, and facilitates early discharge from inpatient wards.
Responsibilities
* Assist in providing a safe environment that supports the physical and mental well-being of patients and staff.
* Deliver evidence-based clinical care with minimal direct supervision.
* Support and participate in joint Health and Social Care Services/interventions, ensuring cooperation and collaboration at all times.
* Ensure patients are treated with respect, and their privacy and dignity are maintained.
* Lead a team of Support Treatment and Recovery Workers in delivering continuous, effective, evidence-based care.
* Carry out comprehensive assessments including risk assessment on admission and at appropriate intervals thereafter.
* Develop and implement individual care strategies with the client, caregiver, and MDT, regularly evaluating and modifying as required.
* Provide high standards of care in line with Trust policies and procedures.
* Understand and contribute to a consistent nursing process and care programme approach, and complete required documentation.
* Carry out clinical procedures and contribute to training of junior staff in these procedures as appropriate.
* Ensure correct administration of drugs in accordance with Trust policy and legal requirements.
* Participate in the Trust observation/engagement policy and monitor patient whereabouts as required.
Qualifications
* RMN
* Post RMN relevant qualification
Experience
* Adult Mental Health Practice
* Community Mental Health Practice
* Working with physical healthcare services
Knowledge
* Ability to use initiative and work without immediate supervision
* Skills in coordinating and planning interagency approaches in mental health care
* An understanding of current mental health issues
Other requirements
