Minimum Requirements:
Licensed to practice medicine in accordance with Chapter, Florida Statutes* plus four years of appropriate experience. Completion of a residency program may substitute for the required experience.
Preferred Qualifications:
13. Certification/License in the state of Florida
14. Federal DEA Certification.
15. Medical Degree – M.D., D.O., or foreign equivalent.
16. Board certified in Family Medicine.
17. Completion of an accredited residency program.
