We currently have 2 openings in our care navigation team at Oak Vale. One is for a 12-month maternity post with the possibility of extending to a permanent position for the right candidate. The second is a full-time/part-time permanent position due to a longstanding care navigator's partial retirement.
Main duties of the job
The role involves working on the front line for the surgery, assisting patients with their healthcare needs.
Main duties include processing prescriptions, scanning documents onto the system, registering new patients, actioning tasks from GPs/nurses, processing online consultation forms, appointment booking/signposting to relevant services, and handling general phone call/front desk duties including answering patient queries.

Oak Vale Medical Centre has been a long-established GP practice providing medical care for generations of families for over 90 years. We pride ourselves on being patient-centred. We are committed to the education of future doctors and have been a training practice for several decades, serving more than 12,500 patients, with the list continuously growing. All salaried doctors were our registrars who returned as GPs after completing their training.
Our team includes GPs, Nurses, HCAs, junior doctors, and in-house Network staff such as an ANP, Pharmacist, and Physiotherapist.
Training is important to us. We have employed apprentices for over 10 years, most of whom go on to secure permanent positions within the practice.
We are high achievers in QOF and are part of a successful Primary Care Network called iGPC.
Job responsibilities
* Working on our front line care navigation team: contact patients via text or letters.
* Actioning tasks from clinicians.
* Handling patient queries on the phone or in person, booking appointments, and signposting.

Person Specification
Experience
* Experience of working in a healthcare setting or primary care.
* Experience of working in GP reception.
* Good interpersonal skills, high attendance and punctuality, patient-centred attitude, and willingness to help.
