You’ll have experience in Adult Social Care and be part of the Social Work Assessment[ Team based in the Borders General Hospital. We are a multi-disciplinary team with both Social Work and NHS working in partnership with service users, carers, family members and external agencies to facilitate discharges from an acute hospital and Intermediate care settings.
