The post holder will be a specialist practitioner and may work across the D2A pathway; bridging between the bed based services and community to support a quality led, seamless service to maximise the patient journey. Assesses service users and formulates, develops, implements and evaluates programmes of treatment /care/support for individuals, families and carers. Registration with HCPC Evidence of postgraduate CPD in at least 2 relevant areas Desirable criteria Modules at Masters level Evidence of further clinical study / specific study relevant to D2A Falls training & related assessments Mental capacity training Member of the Chartered Society of Physiotherapy Member of relevant clinical interest group Experience Essential criteria Relevant experience to the role in a variety of specialties including neurology, orthopaedics, older peoples rehabilitation, falls management, respiratory care, musculoskeletal including assessment and management of various conditions within the area of clinical speciality. Working in a multidisciplinary team. Ability to manage time and caseload effectively. Supporting service users, families and carers in dealing with issues within the area of clinical speciality. Previous experience of working within the NHS as a Physiotherapist Experience of supervising others. Desirable criteria Experience of negotiation and conflict management in clinical situations. Experience of risk management including risk assessment, action planning & evaluation Skills Essential criteria Evidence of specialist clinical knowledge relevant to the post. Understanding of Information Governance, Data Protection, HCPC /CSP standards and Safeguarding Vulnerable Adults/Children. Awareness of relevant NSF’s and NICE guidance. The necessary competences required to undertake this role would be provided (once ratified) as a separate document on appointment.
