We’re looking for a Senior Support Worker to join our Rotherham Hospital Discharge service. The service offers comprehensive housing‑related support to adults on inpatient wards at Swallownest Court. This initiative is driven by our mission to facilitate successful reintegration into the community.
We're in search of an energetic, compassionate, and exceptional Senior Support Worker, who is driven by the desire to make significant and lasting differences in people’s lives. This is an opportunity to be part of something meaningful, transformative, and truly life‑changing.
Key Responsibilities
* Coordinate the daily operation of the service.
* Serve as a Keyworker for a dedicated caseload, providing support in both hospital and community settings, including within people’s homes.
* Promote social justice and make a positive impact in the community.
Qualifications & Requirements
* Hold a full UK driving licence and have access to a vehicle.
* Job subject to an enhanced check made by the Disclosure & Barring Services, paid for by P3.
