The Adult Social Care Review Team handles reviews and reassessments for adults, working with recently discharged hospital patients and community residents. The team also supports various adult social care projects and collaborates with care providers and quality improvement teams, emphasizing outcomes-focused and legally literate practices. We seek practitioners eager to develop their skills and apply their knowledge in a CPD-oriented environment, especially those with experience working with older adults.
The role involves promoting the wellbeing of adults with care needs and improving their outcomes through diverse work and community engagement.
Requirements:
* Social Work England registration
* Relevant social work qualification
* Evidence of Continuous Professional Development
* Driving license and access to a car for work purposes
Employment is contingent upon successful interview, references, DBS clearance, and occupational health checks.
The Council offers visa sponsorship for this role under the skilled-worker route; applicants must be registered with Social Work England and require sponsorship to work in the UK.
