Family Hub Practitioner 0-19 - Gravesham

Role Responsibility

Family Hubs are part of Kent County Council’s support services for children and young people aged 0-19 (SEND 25) and their families.
Our local Family Hubs offer a broad and integrated range of early help services, including midwifery, health visiting, parenting support, and activities for children and young people.
We have 56 Family Hubs across Kent, working alongside partners to support local communities.
An exciting opportunity has arisen to join the newly formed Family Hub team in Gravesham District. The successful candidate will operate as a Family Hub Practitioner across various outreach settings, including home visits, providing 1:1 and group support to children, young people, and their families. The role involves working closely with the integrated children’s services team and multi-agency partners such as schools, police, district council, and community organizations to support vulnerable children, young people, and families.
The post will facilitate and support peer groups and promote the Family Hub Digital and Virtual services, offering advice and guidance. The practitioner will provide expert guidance on baby and child development to help parents support their child's growth, health, and educational outcomes.
You will be expected to work flexibly across the 0-19 age range.
The role requires a whole-family approach and flexibility to work around family schedules, including occasional evening and weekend work, coordinated by management as needed.
This position is customer-facing, and the Council has a statutory duty under Part 7 of the Immigration Act (2016) to ensure candidates have sufficient spoken English/Welsh for effective job performance, as detailed in the Job Description/Person Specification.
The role is subject to an Enhanced Disclosure from the Disclosure and Barring Service.
