Key Duties To be part of the practice team and support the delivery of Herschel Medical Centre and CSN PCN's ambitions to the highest quality and safety.
The role of pharmacist will include but not be limited to:
Minor Illness
QOF contribution and Enhanced Services
Quality Improvement Prescribing Indicators (QOF)
Structured Medication Reviews (SMR)
Long-term conditions (LTCs) management
Care Home- Housebound Medication Reviews
Complex Polypharmacy management
Elderly and frail patient's care
Specialist expertise of medicines
Lead on person-centred Medicines Optimisation
Work with Pharmacy teams across Primary Care, Community and Hospital as part of the wider healthcare team to improve patient outcomes
Manage the repeat prescribing re-authorization process by reviewing requests for repeat prescriptions
Participate and deliver education and training to practice staff on medication-related topics at patient group meetings or other appropriate events, providing advice on the appropriate use of medicines
Produce patient information leaflets and posters and run medicine awareness projects throughout the practice
Facilitate good working relationships between the practice, Community Pharmacies, and Hospital

£58,195 per annum

Key Skills and Qualifications Qualified Pharmacist & Registered with GPhC
Independent prescriber
Minor Illness qualification
Excellent clinical knowledge
Experience of undertaking medication reviews with patients
Excellent Knowledge of Microsoft Excel and Word
Excellent interpersonal and communication skills both verbal and written
Strict confidentiality adherence
Presentable and flexible approach to work
