Overview
As key members of our care team Senior Carers support the Care Coordinator and Team Leader in the planning and delivery of high quality, person- centred care and support to older and vulnerable adults through effective engagement with our service users and mentoring our frontline carers team.
The role requires excellent verbal and written communication skills, strong organisational ability and real commitment to leading and managing by example.
Key Responsibilities
Working closely with the Care Coordinator and Team Leader:
* Meet new service users and their families and carry out initial and risk assessment care planning
* Lead on discussing and agreeing care plans with service users and families
* Carry out Workplace Visits to check all procedures and care disciplines, including evidencing in diary books, being adhered to by frontline carers
* Conduct care plan review meetings with service users and families
* Collaborate with multidisciplinary teams
* Distribute and resupply staff PPE
* Ensure diary books are always available
* Support and encourage frontline carers, whilst reporting any poor practice and making suggestions
Core shifts: 7.30am- 1.15pm and 3.45pm- 9. 15pm; working for some of every second weekend.
On Call Duty. Every 4 weeks 'out of office hours' phone and staff management duties to ensure pro- active continuity and consistency of care - with additional payment of £200.
We are now seeking 2 compassionate and dedicated Senior Carers. The ideal candidates will possess a strong background in older people's care, with a focus on promoting independence and delivering agreed service users' Outcomes.
Experience
* Proven experience in a home care or care home environment, specialising in senior or dementia care
* Knowledge of social care terminology, medication administration, and Care Inspectorate regulatory requirements and standards
* SVQ Level 3 in Health & Social Care, or working towards
* Experience in dementia and challenging behaviour management, and developing personalised care plans is highly advantageous
* Demonstrated ability to move and assist safely and using mobility support equipment
* Strong caregiving skills combined with excellent communication abilities in caring for older and vulnerable individuals
* Full driving licence and access to a car required
* 30 minute maximum commute to work in East Renfrewshire
