To work autonomously in the community setting under the direction of a registered practitioner within the integrated multi-professional ESD and neuro rehab team.
• To be responsible for assessing patient's physical and functional needs, interpreting, planning and implementing therapy interventions for patients with stroke and neurological problems.

Educated to minimum GCSE level or equivalent grade A-C, including English
RSA/OCR stage II - NVQ Level 3 in health and Social Care or equivalent experience
GCSE or equivalent qualification in Math's, Biology and Food Technology
'A' level qualification or other post ideally in healthcare
Experience of working in rehabilitation in health/social care environment at band 3 level or equivalent
Experience / knowledge of rehab principles
Has worked as a Rehabilitation Assistant in the past
Able to demonstrate commitment to high quality care and service provision
Basic computer skills
Must have access to a personal car and the ability to drive independently around the locality, due to the radius covered and lack of public transport in some area
Able to work shifts and unsocial hours when required including flexibility to work over seven days per week
Willingness to embrace integrated model and new ways of working being flexible in approach and attitude
