Role Overview

We are a Mid Sussex charity that supports families, providing clubs and activities for people with learning disabilities, including those with complex needs.

With a growing waiting list and an ambition to expand our service by 30%, we're looking for a Complex Care Coordinator to join our team.

Responsible for leading one of our Kangaroos clubs, STARS, and supporting STARS members when they attend other clubs. Our STARS members have varying needs including severe learning difficulty and/or complex medical needs such as epilepsy, tube feeding, oral suctioning or oxygen dependency. You'll work closely with our Complex Care Nurse Lead to make sure sessions are safe, welcoming, and enjoyable for all.

This is a brilliant opportunity for someone who has experience supporting people with medical or high support needs and wants to work in a rewarding, fun and inclusive environment.

Who are we looking for?

We're interested in hiring the right person for this role, so please do not be put off if you do not match all of the criteria. If you're passionate about making a difference, confident in managing other members of staff within a service delivery setting and feel you could excel in this role, we would love to hear from you.

Perhaps you're a former or student nurse, a healthcare support worker, a teaching assistant with experience in SEN, or someone with lived or professional experience supporting people with complex needs. We'd especially welcome applications from those with a medical or clinical background.

Main Duties

* Lead sessions for our STARS members including carrying out delegated duties as described above, following care plans and emergency action plans, ensuring safety and compliance.
* Work with the Complex Care Nurse Lead to create a diverse, safe and enjoyable programme of activities and opportunities, designed by and for members to achieve positive outcomes within financial and other parameters agreed with Head of Services.
* Support the Complex Care Nurse Lead to prepare training materials, updating documentation and monitoring the quality of care delivery
* Support the Complex Care Nurse Lead with the administration of staff inductions and competency sign-offs
* Support the Head of Service & Complex Care Nurse Lead to ensure compliance with appropriate legislation, e.g Ofsted / CQC
* Work closely with the Complex Care Nurse Lead to implement and follow care plans and risk assessments
* Maintain a warm and caring environment, ensuring safe, creative, and challenging activities chosen by members meet their needs, interests and strengths
* Build and maintain effective working relationships with relevant stakeholders, including members, parent carers, venue management, activity providers, ensuring suggestions/concerns are responded to clearly and sensitively
* Ensure all administration is accurate, up to date and completed to relevant standards and legislation. (member care plans, activity plans, registers, accident/incident records, safeguarding records, health and safety checklists, daily debrief records and procedures for administering medical treatment)
* Attend Family Support sessions where appropriate to support families including Parent Carers & Siblings of STARS members, and attend Early Years sessions where appropriate to meet prospective STARS members.
* Support the team in running activities in the most effective and efficient manner
* Be a positive role model, embedding Kangaroos values, demonstrating appropriate professional behaviours and ways of working
* Fulfil any other duties considered reasonable as directed by the Complex Care Nurse Lead and senior management.
* Actively promote equal opportunities and challenge discrimination.
