Overview
We are currently recruiting a dynamic Social Worker to work within a community mental health service in West Dorset. The role is based in an integrated team in a community setting with health colleagues, supporting people with significant mental health needs and transitions to adulthood. The service operates within a recovery model, promoting strengths-based and person-centred practice to support independence and enable individuals to achieve their best outcomes. The organisation supports attendance on the Approved Mental Health Professional training and offers regular reflective supervision to support Continuous Professional Development and a robust learning and development offer.

Responsibilities
* Undertake strengths-based assessments, support planning and reviews to help customers achieve their outcomes and promote independence within a recovery model.
* Undertake Mental Capacity Assessments.
* Work with people subject to the Mental Health Act and complete S117 assessments and reviews.
* Co-locate and work alongside health colleagues including Community Mental Health Nurses, OTs, Psychologists and Psychiatrists to provide services for adults with Serious Mental Illness or Autism Spectrum Condition.
* Collaborate with other health and social care professionals.
* Identify carers’ needs and support their assessments.
* Take opportunities to take leadership roles and support service development.
* Demonstrate a passion for working with people with significant mental health problems and promote recovery.
* Display excellent communication skills, adaptability to a fast-paced environment, and the ability to work with a range of professionals.
* Be creative and passionate about enabling people to be as independent as possible.
* Be confident with the legal basis for practice and demonstrate understanding of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 2007, Human Rights Act 1998 and Equality Act 2010.
* Demonstrate commitment to continuous professional development and hold a degree in Social Work or equivalent.
* Fulfil the travel requirements of the post.
Qualifications
* Degree in Social Work or equivalent.
* Knowledge of Care Act 2014, Mental Capacity Act 2005, Mental Health Act 2007, Human Rights Act 1998 and Equality Act 2010.
* Excellent communication skills and ability to work with multi-disciplinary teams.
* Willingness to participate in AMHP training and development opportunities.
