Offer description

Responsibilities

* Arrange meetings
* Take minutes in meetings
* Ensure actions from meetings are completed in a timely manner
* Send out meeting agendas and associated papers/documents
* Act as a contact point for the PCN
* Send/receive e‑mail's on behalf of the Clinical Director and PCN Strategic Manager where appropriate
* Complete actions for Clinical Director and PCN Strategic Manager where appropriate
* Keep Teams groups and documents updated
* Provide admin support to other leadership positions within the PCN where required
* Work closely with the Social Prescribers and Health and Wellbeing Coaches
* Support the PCN leadership team to look at population health intelligence to help proactively identify and deliver personalised care
* Work with the GPs and other primary care professionals within the PCN to help support them in identifying and managing a caseload of patients, and where required and as appropriate, refer people onward to other health professionals within the PCN
* Raise awareness within the PCN of shared decision‑making and decision support tools
* Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to have shared decision‑making conversations
* Use EMIS to support adding rotas, making changes to templates and organising/booking patients where required across the PCN

* PCN Clinical Director
* PCN Strategic Manager
* PCN Operations Manager
* Practice Managers
* General Practitioners and other practice clinicians
* SWGP Director of Planning and Delivery
* ICB
* Patients & public stakeholders, including patient participation groups
* Relevant representative committees (e.g. LMC, LDC, LOC, LPC)
* Healthcare providers including NHS, third sector and voluntary providers


Professional Development and Review

A personal development plan will be agreed each year with the post holder to further develop the skills and knowledge required to carry out the responsibilities associated with this role.
