Shifts: Long Days (typically 08:00–20:00), 7-day rota incl. weekends

About the role
We're launching a specialist Substance Misuse support package for adults in the community. As a Support Worker you'll deliver person-centred care that supports harm reduction and recovery, working alongside local services and families.
What you'll do
* Provide day-to-day support in people's homes and the community
* Follow recovery and relapse-prevention plans; promote harm-reduction strategies
* Support with routines, daily living skills, wellbeing check-ins, and appointment attendance
* Prompt medication as per care plan; record accurate notes and incident reports
* Work safely when lone-working and escalate safeguarding concerns promptly
* Liaise with multidisciplinary teams (commissioners, keyworkers, clinicians)
About you
* Experience supporting people with substance misuse/dependency (preferred but not essential)
* Confident managing risk, boundaries, and de-escalation
* Clear written and verbal communication; reliable and professional
* Enhanced DBS (or willing to apply) and Right to Work in the UK (essential)
* Driving licence & access to a car (preferred but not essential)
