An exciting opportunity has arisen for a highly motivated individual to join our Community Nursing Team, covering the North West locality of Shropshire, providing planned care to patients in the community.
Main duties of the job
* You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development.
* We also actively encourage newly qualified nurses to join our team. In return we offer a preceptorship programme and support professional development in a nurturing environment, allowing you to reach your full potential.
* The team provides nursing care to patients within their own homes or chosen place of residence offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential.
* We are committed to clinical support, CPD & training. The post is covering 7 days, 8am to 6pm service.
Qualifications
* RGN NMC Registered.
* Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes.
Experience
* Experience of working in a multi-disciplinary / multi-agency health/social care environment.
* Experience and knowledge of basic administration systems including Microsoft Office.
* Experience of successful partnership working arrangements with other services.
* Previous community experience.
Knowledge and Skills
* Up to date with Mandatory Training.
* Uses evidence based practice.
* Works within own defined competencies.
* Awareness of changing healthcare factors influencing change.
* Well refined communication (verbal and written), interpersonal skills - ability to gain rapport at all levels.
* Engaging and empowering approach that seeks to work with others and deliver outcomes in a successful and well informed manner.
* Demonstrates tact and diplomacy.
Other
* Must be able to meet the transport requirements of the post.
Disclosure and Barring Service Check
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
Location: North West IDT Team, Cambrian Medical Centre, Thomas Savin Road.
Salary: £32,073 to £39,043 a year (pro rata).
Contract: Permanent.
