Job summary

An exciting opportunity has arisen to work as a Band 5 Community Nurse for the Virtual Ward within the Central Cheshire Integrated Care Partnership Care Community Teams.

Working in partnership with Care Community Teams, Primary Care Services and Mid Cheshire NHS Foundation Trust to ensure the smooth and timely enrolment of patients deemed suitable to receive safe care and treatment in their own home. The post holder will be part of a team responsible for the care and support of all Virtual Ward patients.

Patients will be remotely monitored by the team, who will also undertake home visits to complete holistic assessments as required.

The Virtual Ward currently centres around patients with diagnoses of frailty, acute respiratory infection and heart failure. There are a high percentage of highly complex patients, offering post holders the opportunity to develop their clinical skills and knowledge.

You will also be joining a team who work to uphold the trust values and behaviours. The post holder will be fundamental in ensuring consistent service delivery and maintaining communication and a joined-up clinical approach across community and secondary care.

The rapid expansion of virtual care models presents an opportunity to join an innovative and growing service. As the service continues to develop, referral pathways and specialist involvement are expected to expand further.


Main duties of the job

To work under the direction of the Virtual Ward Team Leader and Specialist Nurses.

To be responsible for assessing and managing the health needs of an identified population. To develop, implement, and evaluate programmes of care for a range of target groups, including patients receiving end-of-life care and those with long-term conditions and complex nursing needs.

To support and wherever possible maintain patients in their own preferred place of care, enabling them to maximise their independenceand optimise their quality of life.

To be expected to hold a designated caseload and deputise in the absence of the Virtual Ward Specialist Nurse.

At times be expected to support the wider community nursing team.

Job responsibilities

To work within the agreed framework for Community Nursing Services To assess, provide and review holistic nursing care, advice andinformation to patients and carers within the home environment or appropriate care setting in a safe and professional manner usingevidence-based knowledge.

Undertake initial assessments on receipt of specific referrals to the caseload, acknowledging the needs of clients and their carers.

Assess, plan and implement and evaluate a plan of care to meet the patients physical, social, emotional and environmental needs,encompassing a rehabilitative/ self-care approach where necessary and as per community nursing contract.

To provide skilled nursing care and ability to undertake clinical tasks including wound care, venepuncture, and catheter care.

To be able to recognise, interpret and analyse clinical data related to the individual clients needs including client observations.

Demonstrate, teach, and supervise the nursing care that can be safely delegated to the patient/carer giving support as necessary,including specialist technological devices, and equipment provided by Virtual Wards.

Promote clinical excellence and evidence-based approach to care by complying with the agreed Trust policies and procedures andmaintaining clinical competencies in line with competency framework.

Work to the End of Life (EOL) Strategy including adherence to all EOL tools, acting as a clinical expert in EOL care provision

Take an active role in health education of the individual and community promoting self-care and providing both planned andopportunistic care based upon evidenced based information.

Accurately record all patient contacts on Emis web and Trust documentation as per MCHFT policy ensuring patient and professionalconfidentiality is always maintained in accordance with NMC guidelines

Manage and assess patients discharge potential and need from the caseload.

Ensure that equipment and aids, necessary for effective care, meet the needs of individual patients and are adequately maintained.

Demonstrate and explain safe use of equipment for patient and/or carer where required.

Participate in the delivery of care involving complex needs and supporting carers and relatives in distressing circumstances.

Refer people to other practitioners when needs and risks are beyond ones own scope of practice or require longer term support.

Prioritising and managing own caseload & client group

Assume the responsibility for the clinical case load in the absence of the Team Leader/Senior Lead delegating work appropriately withinthe competency framework

To participate in the measurement and monitoring of quality standards as appropriate.

To maintain accurate personal and professional records adhering to MCHFT and Nursing and Midwifery Council policy and guidelines.

To ensure the most efficient and cost-effective utilisation of all resources (human, physical and financial).

To report accidents/incidents of patients or self which occur whilst on duty and to complete on the Incident reporting systemimmediately. To participate in any investigations as necessary.

To contribute to and co-operate in the co-ordination of off duty and holiday leave with colleagues, ensuring service cover is maintained.Working effectively to support other team members in all aspects of the service.
