We are seeking a motivated and enthusiastic Occupational Therapist or Physiotherapist to join our team to provide clinical advice and guidance within the care home setting to support early identification of increasing therapeutic and health needs, providing a clinical link between the care home setting and community health services to support appropriate referral and intervention.
The successful candidate will work in line with the Enhanced Health in Care Home (EHCH) principles, taking a pro-active approach to workforce development and early identification of escalation of need to support ageing well and quality of life for individuals in a care home setting.
You will deliver FALLS prevention and early identification of body shape changes due to positioning training & education to care home staff adopting a variety of educational methods; for example, floor walking, on the job learning, formal & informal presentations, Q&A sessions, presentations to care provider forums, develop information resources suitable for care home skill mix team members.
You will ensure early identification, assessment & intervention of care home residents for therapy input, with independent management of own caseload. This will include working in partnership with Therapy Leads & Care Home Team leads to analyse data to determine clinical interventions, writing up, reporting and evaluating interventions on a monthly basis.
You will provide clinical leadership within therapy hubs, across the service line, to promote proactive clinical & educational interventions with care homes.
The post holder will work autonomously across all care home settings within Leicester, Leicestershire and Rutland and will empower residents to age well within their care home environment, working across neighbourhood systems.
You will have the clinical expertise and communication skills to be able to identify areas of good practice and to effectively challenge areas for improvement as part of training interventions. This will include collating data to identify trends and themes for further evaluation with Care Home Team leads.
You will create a community of practice for ongoing support across LLR around upskilling care home workforce in identified key areas using a robust process, ensuring ongoing competency and quality of training delivery to share good practice and discuss challenges.

Job Description Summary (Main duties and responsibilities)
* To promote the independence and well-being of older people and vulnerable adults in line with LLR Care Home Sub Group Priorities.
* To provide clinical oversight and advice around appropriate escalation and referral to appropriate services within CHS for identified individuals, to enable timely input and prevention of harm.
* To work autonomously with a solution focused approach, utilising hard / soft data and resources available across the system to identify and prioritise the care homes for intervention.
* Create data base to record interventions and to identify champions within the homes that are trained and refresher schedules.
* To liaise with CHS, LA and ICB, and use data collection to identify key care homes for targeted intervention, and provide feedback as required.
* To collate feedback following the training intervention and forums, to participate in evaluation and ongoing development.
* To ensure training content is contemporaneous and in line with current standards and guidelines and update as required.
* To maintain professional development through engagement with clinical supervision and training opportunities.
* To work with the care home to analyse FALLS, conveyance to acute hospital settings, and referrals into Therapy. To evaluate data following clinical intervention and obtain care home feedback.
