Join to apply for the Advanced Frailty Practitioner role at NHS Borders. We have an exciting opportunity for a highly motivated and innovative Occupational Therapist/Physiotherapist to join our newly formed Frailty Unit in NHS Borders. You will be instrumental in the continuing development of this new post in a supportive, established AHP Team. You will have excellent leadership, communication and interpersonal skills and demonstrate experience across the NMAHP Pillars of Practice (Clinical, Leadership, Facilitating Learning, Evidence/Research & Development).
We have a 7-day service (working 5/7) with the expectation of extended hours in the future. The post is based at the Borders General Hospital and involves collaboration with the Emergency Department multidisciplinary team to deliver high quality care while supporting early frailty assessment and discharge planning where possible.
Responsibilities
* Apply advanced clinical judgement and decision making within the Emergency Department, Frailty Unit and Medical Assessment Unit, providing early frailty assessment with a focus on prevention of admission, and setting goals and functional criteria for discharge early in the patient journey.
* Support the development of the new Frailty Unit post within a supportive AHP Team and contribute to service planning with accurate data collection.
* Demonstrate excellent leadership, communication and interpersonal skills and apply knowledge across the NMAHP Pillars of Practice (Clinical, Leadership, Facilitating Learning, Evidence/Research & Development).
* Engage in ongoing personal and professional development to provide a quality service, supported through CPD, appraisals and in-service training.
* Work autonomously and be able to make decisions under pressure while understanding the whole system flow.
* Be part of a 7-day service with potential for extended hours in the future.
Qualifications / Experience
* Highly motivated and innovative Occupational Therapist/Physiotherapist with experience in frailty and acute care settings.
* Excellent leadership, communication and interpersonal skills.
* Experience across the NMAHP Pillars of Practice (Clinical, Leadership, Facilitating Learning, Evidence/Research & Development).
* Forward thinking, proactive with strong organizational and clinical skills.

Please note: The salary quoted is for full-time hours (37 hrs per week). For appointments to part-time hours, the salary will be pro-rata.
