Job summary

Advanced Practitioner in Acquired Brain Injury, Band 8a (22.5 hours a week)

This is a 3-day (22.5 hours) Band 8a Advanced Practitioner role based in the Brain Injury Service, covering two acute hospitals and community settings within Gloucestershire. The role involves delivering assessment, rehabilitation, and management for brain-injured patients with highly complex needs, including those in a disorder of consciousness. The AP will assess individual patient needs, initiate referrals, determine appropriate evidence-based treatments, and facilitate efficient patient pathways.

The position involves both inpatient and outpatient care. The AP will provide advice, support, education to colleagues, and act as a clinical role model and educator to develop and deliver responsive care.

Additional working pattern details: Mon/Wed/Fri.


Main duties of the job

The post holder will:

* Support the flow of ABI patients within the acute hospital and assist the multidisciplinary team with complex cases.
* Deliver highly complex care elements requiring advanced clinical or technical skills, including independent assessments and management of patients, considering physiological, psychological, and pharmacological factors. Discharge patients when appropriate.
* Provide leadership and continuity for complex patients.
* Facilitate timely patient movement across services by liaising proactively with wards, departments, rehabilitation units, and community settings, ensuring expert inter-professional care.
* Participate in research to inform clinical knowledge and enhance the role of advanced practitioners.
* Act as a change agent, recognizing service needs and contributing to service development.

Job responsibilities

* Support the multidisciplinary team with complex patients, delivering advanced assessments and management, including discharges, with accountability for decisions.
* Provide evidence-based care and develop multi-disciplinary treatment plans across the system.
* Offer expert clinical advice and leadership within the team.
* Facilitate patient movement and care pathway management proactively.
* Independently refer and discharge patients appropriately.
* Lead and ensure continuity of care for complex patients, including communication with families and participating in relevant meetings.
* Participate in incident reviews, investigations, and quality assurance processes as an expert adviser.
* Understand your scope of practice, including managing risk and uncertainty, and refer to senior clinicians when needed.
* Apply advanced clinical expertise to influence practice, reduce variation, and promote best practices.
* Develop professional relationships within multidisciplinary teams and across services to ensure seamless care pathways.
* Contribute to standards development and service improvement initiatives.


Person Specification


Qualifications (Essential)

* MSc in Advanced Practice from an accredited pathway or willingness to undertake NHSE ePortfolio route.
* Formal assessment of practice capabilities by an occupationally competent assessor.
* Evidence portfolio against the HEE 2017 Multi-professional Framework.
* Registered Healthcare Professional with a regulatory body.


Experience and Knowledge (Essential)

* Extensive clinical experience in Acquired Brain Injury.
* Experience in teaching, supervision, or mentorship.
* Expertise in direct patient care with high-level diagnostic and assessment skills.


Desirable

* Research leadership experience as Principal Investigator.
* Experience in developing practice guidelines and governance.
