Overview
We are looking for an enthusiastic and dynamic Registered Health Care Professional to join a well-established Trusted Assessor Team within Hull and East Riding Intermediate Care. The role requires the post holder to competently assess patients’ suitability for rehabilitation following inpatient hospital treatment. The role also involves the assessment of patients who may not be managing in their home environment and require rehabilitation within a bedded unit facility.
There is the requirement to work between CHCP Bases as required and in reach into Hull Royal Infirmary and Castle Hill Hospital. As a Trusted Assessor you will play a vital role in the Intermediate care model. This involves closely working with system partners to optimise patient care.
Responsibilities
* Assess patients’ suitability for rehabilitation after inpatient hospital treatment.
* Assess patients who may not be managing in their home environment and may require rehabilitation within a bedded unit facility.
* Work between CHCP Bases and in reach into Hull Royal Infirmary and Castle Hill Hospital as required.
* Play a vital role in the Intermediate Care model by working closely with system partners to optimise patient care.
* Be part of a supportive multi-disciplinary team and participate in regular supervision; the Intermediate Care team supports development opportunities.
* Demonstrate a broad knowledge base, good time management skills and relevant experience; maintain excellent communication and collaborate with staff from all disciplines while upholding high professional standards.
* Work as a Trusted Assessor within Intermediate Care, a 7 day, 8am to 8pm service, and participate in the weekend rota.
* Travel between community bedded units and patients’ homes in and around Hull; have access to suitable transport.
Qualifications and qualities
* Broad knowledge base relevant to Trusted Assessor duties.
* Good time management and the ability to work collaboratively with multidisciplinary teams.
* Excellent communication skills and high professional standards.
